When submitting in documentation to the board for application or upgrade you must submit the most current forms which can be found on our website at the “Applications and Forms “ tab.
All application materials must be sent to the board as a single packet which must include the board required forms, fee, transcript, and proof of passing the National Counselor Exam (NCE). (Including Transcript In Packet Is Optional, But Recommended). Incomplete application packets will be returned to the applicant without review. Application fee will be retained by the board and is not refundable. The applicant may resubmit a complete application packet within 45 days of the date of the notice of deficiency without additional fee. However, if the resubmitted application packet is received after the 45 day requirement, the original application fee is forfeited and the applicant must submit a new application fee of $98.00 along with the application material.
