Health Care Provider Enrollment

Learn more about the Children with Special Health Care Needs (CSHCN) Services Program online, at your own pace and at any time by visiting Training for Providers. This free online training will take you through the steps for enrollment in our program and what you can expect as a program provider.

In 2011 the CSHCN Services Program Provider Manual became an online document that is updated monthly. It is available on the Texas Medicaid and Healthcare Partnership (TMHP) website as a complete manual or as individual chapters. 

How to Enroll

Enrolling in the CSHCN Services Program is now a click of the mouse away! Go to the Enrolling in the CSHCN Services Program page of the TMHP website. You can enroll online or print out the paper application form to complete and return to TMHP. If you have questions, call your local area TMHP Provider Relations representative. To find your area representative, go to the Provider Enrollment Regional Support web page or call 1-800-568-2413.


Family Support Services Provider Enrollment

If you are a respite, home modification, vehicle modification, home inspection, and adaptive aid and special equipment provider doing business in Texas, you may apply to become a CSHCN Services Program Family Support Services (FSS) provider.

How to Enroll

Call the Toll-Free Help Desk at 1-800-252-8023 or email cshcn@dshs.state.tx.us to request the Family Support Services Provider Enrollment packet. Home inspectors only also need to provide a copy of Real Estate Inspector License through the Texas Real Estate Commission.
