The information in this section is intended to provide public notice of disciplinary action by the Department of State Health Services and the Division for Regulatory Services, Enforcement Unit and is not intended to reflect the specific findings of either entity.

This information may not reflect any number of factors including, but not limited to, the severity of harm to a patient, any mitigating factors, or a provider/certificant's disciplinary history. This listing is not intended as a guide to the level of sanctions appropriate for a particular act of misconduct.

Easter, Ryan P., Corpus Christi, TX. April 18, 2016, 3 day suspension for violating EMS Rules 25 TAC §§157.34(e), 157.36(b)(15), and 157.36(b)(18) related to falsifying renewal application regarding CE compliance.

May 23, 2016, 5 day suspension for violating EMS Rules 25 TAC §§157.36(b)(2), 157.36(b)(6), 157.36(b)(22), 157.36(b)(23), 157.36(b)(26), 157.36(b)(28) and 157.36(b)(30) related to arrests and conviction for violation of a protective order, deferred adjudication for aggravated assault-family member, and failure to notify the Department of  arrests and of a change in criminal history.

August 3, 2015, assessed a $21,315.00 administrative penalty for violating EMS Rules 25 TAC §§157.11(a)(2), 157.11(c)(2)(D), 157.11(d)(1), 157.11(i)(2), 157.11(j)(1), 157.11(j)(5), 157.11(m)(11), 157.11(m)(16),  and 157.11(m)(31) related to failing to have EMS ambulance vehicle(s) adequately equipped and supplied at all times, failing to display vehicle authorization in patient compartment, failing to be in compliance with federal  and state laws and regulations, failing to have a spare battery or alternative source of power for critical patient care equipment, and failing to provide provider application fee and vehicle fees per vehicle. 
Floren, Casey D., Aubrey, TX. October 1, 2014, revoked for violating EMS Rules 25 TAC §§157.36(b)(2), 157.36(b)(14), 157.36(b)(19), 157.36(b)(23), 157.36(b)(28), 157.36(b)(29) and 157.36(b)(30) related to misappropriating and/or tampering and/or adulterating, and/or self-administering morphine from his employer and an arrest for diversion of controlled substance for personal use and not notifying the Department of the arrest.

Freed, Christopher R.  DBA North Texas EMS, Springtown, TX. February 5, 2016, assessed a $500.00 administrative penalty for violating EMS Rules 25 TAC §§157.11(m)(8) and 157.16(b) related to failure to make accurate and complete PCR and failure to provide the Department with requested information. 
Fuentes, Christina C., Arlington, TX. October 1, 2014, reprimanded for violating EMS Rules 25 TAC §§157.36(b)(7) and 157.36(b)(30) related to failing to follow medical director protocols by performing advanced level of care outside your scope of certification.

February 26, 2016, 12 month probated suspension for violating EMS Rules 25 TAC §§157.43(h)(2), 157.43(h)(3), 157.43(h)(4), 157.43(h)(9), 157.43(h)(10), 157.43(h)(11), 157.43(h)(14), 157.43(h)(15), 157.43(h)(16), 157.43(m)(3)(B), 157.43(m)(3)(G), 157.43(m)(3)(K), and 157.43(m)(3)(T) related to starting an EMT class without approval from the Department, failure to verify student file prerequisites and course requirements, and failure to document preceptors were trained, clinical/internship objectives were met.
