This information may not reflect any number of factors including, but not limited to, the severity of harm to a patient, any mitigating factors, or a provider/certificant's disciplinary history. This listing is not intended as a guide to the level of sanctions appropriate for a particular act of misconduct.

October 6, 2017, assessed a $500.00 administrative penalty for violating EMS Rules 25 TAC §§ 157.11(g)(9), 157.11(m)(3), 157.11(m)(8), 157.11(m)(12), 157.11(m)(32), 157.16(c), 157.16(d)(11), 157.16(d)(12),  and 157.16(d)(19) (Rules in effect on the date of occurrence) related to failure to assure accuracy of PCRs, implement and enforce policy, procedure and protocols, billing for level of care not provided, failure to perform adequate internal investigation of a complaint, and failure to provide requested information to the Department.
Agamah-Hihewodo, Koffi M., Houston, TX.  April 23, 2018, denied renewal application for EMT certification for violating EMS Rules 25 TAC §§157.34(a)(3), 157.36(b)(2), 157.36(b)(4), 157.36(b)(14), 157.36(b)(18), 157.36(b)(22), 157.36(b)(23), 157.36(b)(29), and 157.36(b)(30) related to fraudulently billing Medicaid and Medicare, an indictment and deferred adjudication for theft-aggregate, failure to notify the Department of an arrest and change in criminal history, and failure to timely submit a renewal application.

Alvarez, Heron, Roma, TX. December 14, 2016, 24 month probated suspension for violating EMS Rules 25 TAC §§157.36(b)(2), 157.36(b)(19), 157.36(b)(28), 157.36(b)(29), and 157.36(b)(30) related to operating a motor vehicle while intoxicated and possessing a controlled substance cocaine. 
Ambu-Care Trans LLC DBA Ambu-Care Trans, Houston, TX.  February 16, 2018, assessed a $3,450.00 administrative penalty for violating EMS Rules 25 TAC §§157.11(n)(1), 157.11(n)(5), 157.11(n)(9), 157.11(n)(22), 157.11(n)(39), 157.16(d)(14), 157.16(d)(17), and 157.16(d)(19) related to failure to ascertain personnel are currently certified or licensed by the Department, develop and implement policies, procedures and protocols necessary for operations to comply with federal and state laws and regulation, and failure to assure patient care reports are completed accurately.
Belton, Derrick, Houston, TX.  March 22, 2018, 10 day suspension beginning April 23, 2018; for violating EMS Rules 25 TAC §§157.33(l)(3), 157.36(b)(1), 157.36(b)(3), 157.36(b)(11), 157.36(b)(26), 157.36(b)(31),  and 157.36(b)(35) related to recklessly operating an ambulance, an arrest for disregard red light and no driver’s license, failure to notify the Department of an arrest, failure to follow medical director’s protocols, and failure to complete patient care reports.

Berry, Ryan E., Palestine, TX. November 30, 2017, revoked for violating EMS Rules 25 TAC §§157.36(b)(24), 157.36(b)(31), 157.36(b)(34), 157.36(b)(35), and 157.36(b)(47) related to submission of inadequate samples for drug screen and refusal to submit another sample, resigned from employment during drug request, and failure to provide requested information to the Department.
Bowser, David B., Lubbock, TX. October 11, 2017, 12-month probation pursuant to 25 TAC §157.36(g) related to reapplication for EMS certification after surrendering previous EMT certification while disciplinary action was pending. 
Brown, John A., Magnolia, TX. October 18, 2017, reprimanded for violating EMS Rules 25 TAC §157.36(b)(2), 157.36(b)(7), 157.36(b)(14), 157.36(b)(28), and 157.36(b)(30) (Rules in effect on the date of occurrence) related to misappropriating supplies from employer, self-insert IV, and failure to properly dispose of expired nitro by spraying it inside the back of an ambulance. 
Calhoun, Steven R., Fort Worth, TX.  April 19, 2018, reprimanded for violating EMS Rules 25 TAC §§157.36(b)(2), 157.36(b)(23), 157.36(b)(25), 157.36(b)(28), 157.36(b)(29), and 157.36(b)(30) (rules in effect on the date of the occurrence) related to an arrest and deferred adjudication for aggravated assault with a deadly weapon and failure to notify the Department of an arrest and change in criminal history. 
Cardec, Carlos J., Kyle, TX.  April 25, 2018, revoked for violating Tex. Health & Safety Code § 773.0614 related to deferred adjudication for aggravated robbery.
Castillo, Jr., Juan Jose, Alamo, TX. Criddle, Larry M., Dickinson, TX.  January 8, 2018, 12 month probated suspension for violating EMS Rules 25 TAC §§157.36(b)(2), 157.36(b)(21), 157.36(b)(22), 157.36(b)(23), 157.36(b)(26), 157.36(b)(28), and 157.36(b)(30) (rules in effect on the date of the occurrence) related to an arrest for driving while intoxicated, conviction for driving while intoxicated 3rd or more, and failure to notify the Department of an arrest and change in criminal history.
Dillow, Kathleen M., Houston, TX.  January 22, 2018, 12 month probated suspension for violating EMS Rules 25 TAC §§157.36(b)(7), 157.36(b)(28), 157.36(b)(30), and 157.36(b)(31) related to failure to follow medical director’s protocols. 
Duncan, Christopher W., Kountze, TX. October 26, 2017, revoked for violating EMS Rules 25 TAC §§157.36(b)(2), 157.36(b)(6), 157.36(b)(26), 157.36(b)(31), and 157.36(b)(35) related to an arrest for continuous sexual abuse of a young child or children, and failure to notify the Department about an arrest. 
Emergency Room Mobile Services LLC DBA Rescue Squad, Flower Mound, TX.  April 17, 2018, 12 month probated suspension and assessed a $500.00 administrative penalty for violating EMS Rules 25 TAC §§ 157.11(n)(26), 157.16(d)(4), 157.16(d)(19),  and 157.16(d)(12) related to operating without medical director oversight, failed to notify the Department of a change of its medical director status, and failing to correct deficiencies. 
Exclusive Medical Transport Services LLC DBA Exclusive Ambulance Service, San Juan, TX. October 18, 2017, assessed a $22,500.00 administrative penalty with $10,500.00 probated for 3 years for violating EMS Rules 25 TAC §§157.11(d)(7), 157.11(j)(1)(C), 157.11(j)(1)(K), 157.11(j)(1)(M), 157.11(m)(1), 157.11(m)(15), 157.11(m)(16), 157.11(m)(20), 157.16(d)(10), 157.16(d)(13) and HSC 773.007(b) related to failure to maintain medical director supervision, failure to notify the Department of a change in medical director’s status, utilizing an ambulance and transporting patients without medical direction, falsified documents to renew Provider license, failing to have EMS ambulance vehicle(s) adequately equipped and supplied at all times, failing to have provider name displayed on both sides of the ambulance, and failing to comply with a Department Order.
Faust, Arthur L., Conroe, TX. November 30, 2017, revoked for violating Tex. Health & Safety Code § 773.0614 related to felony conviction and imprisonment for improper photography. 
Fancher, Travis W., Pflugerville, TX. Fisher, Jason D., Hurst, TX.  May 18, 2018, reprimanded for violating EMS Rules 25 TAC §§157.33(l)(3), 157.36(b)(3), 157.36(b)(4), 157.36(b)(24), and 157.36(b)(47) related to failure to follow employer’s policies, falsified PCRs, forged signature on PCR, and failure to provide requested information to the Department.
Ford, John H., Fort Worth, TX.  July 31, 2018, revoked for violating EMS Rules 25 TAC §§ 157.36(b)(21), 157.36(b)(23), 157.36(b)(24), 157.36(b)(26), 157.36(b)(27), 157.36(28), 157.36(b)(30), 157.36(b)(31), and 157.36(b)(35) related to arrests for aggravated assault with a deadly weapon, abandon/endanger child imminent danger, and harassment by threatening text messages, deferred adjudication for Aggravated Assault with a deadly weapon and abandon/endanger child imminent danger, failure to notify the Department of arrests and change in criminal history, and failure to provide requested information to the Department.

Lee, Joshua D., Houston and/or Montgomery, TX.  June 27, 2018, suspended for three months to begin July 27, 2018 for violating EMS Rules 25 TAC §§157.36(b)(2), 157.36(b)(21), 157.36(b)(24), 157.36(b)(27), 157.36(b)(29), and 157.36(b)(30) (rules in effect on the date of the occurrence) related to a conviction for misprision of a felony, knowledge of fraternity brothers selling marijuana and failure to notify proper authorities, and failure to provide requested information to the Department.

January 23, 2017, 5 year suspension; 5 days actual suspension followed by 4 year 11 month 25 day probated suspension for violating EMS Rules 25 TAC §§157.36(b)(2), 157.36(b)(14), 157.36(b)(18), 157.36(b)(22), 157.36(b)(23), 157.36(b)(26), and 157.36(b)(30) related to conviction for theft of property, an arrest for misapplication of fiduciary finance/property, failure to notify the Department of arrests and convictions.

December 13, 2017, 12 month probated suspension for violating EMS Rules 25 TAC §§157.36(b)(3), 157.36(b)(4), 157.36(b)(6), 157.36(b)(7), 157.36(b)(8), 157.36(b)(11), 157.36(b)(31), 157.36(b)(35), 157.36(b)(36), 157.36(b)(37), and 157.36(b)(41) related to failure to prevent partner from inappropriate conduct with patients, failure to follow medical director protocols by allowing partner to perform advanced level of care, failure to fully document PCRs, and failure to maintain substantial amount of skill to be an assertive lead medic. 
Nwako, John A., Houston, TX. March 8, 2017, 7 year probated suspension for violating EMS Rules 25 TAC §§157.36(b)(2), 157.36(b)(14), 157.36(b)(18), 157.36(b)(22), 157.36(b)(23), 157.36(b)(26), and 157.36(b)(30) related to an arrest and conviction for theft involving Medicaid and Medicare billing and failure to notify the Department of an arrest and conviction.
