ImmTrac is the Texas immunization registry, a free service provided by the Texas Department of State Health Services (DSHS). It is a secure, confidential registry that stores vaccine information electronically for Texans of all ages.

ImmTrac is designed to consolidate immunization records from multiple sources, and offers many benefits to health-care providers and their patients. ImmTrac allows providers easy Internet access to immunization histories on participating patients, and offers Reminder and Recall capability for children 17 and younger. ImmTrac is available at no cost to authorized health-care providers.

To register with ImmTrac as an authorized ImmTrac provider, please complete the online ImmTrac Site Registration Form. Below are links to some provider educational materials, including brochures, fact sheets, and posters.
