2024 Local Registrar Self-Assessment Survey

The 2024 survey is due April 30, 2024.

To complete the Self-Assessment survey, you will need:

  • Knowledge of office policies and procedures
  • Ability to view your birth/death indexes
  • Numbers for births/deaths registered and birth/death certified copies issued
  • Names and TxEVER User IDs of vital record employees
  • Contact information for any branch/annex locations.

Send the survey and necessary attachments in a single email to fieldservices@dshs.texas.gov. These include the documents listed at the bottom of the self-assessment survey once it is completed.

Contact Field Services at fieldservices@dshs.texas.gov if you have questions, technical difficulties, your response did not save, or if you need a copy of your submitted response.

Note: If we do not receive your survey by the due date, you are out of compliance with Section 191.022(g) of the Texas Health and Safety Code.

TxEVER Criminal Background Checks

DSHS has begun a phased implementation of criminal background checks (CBC) for users who access TxEVER, per Health and Safety Code (HSC) §191.071. The HSC sets forth requirements for fingerprint-based criminal background checks for these users.

Remote Birth Issuance

The TxEVER Fee Module Remote Birth issuance is the same process used by the state.

The following process should be completed every time the module is used.

  1. Customer accurately and completely fills out and submits request application.
  2. Customer submits valid government-issued photo identification.
  3. The local registrar's office reviews the application for accuracy, completeness, and eligibility.
  4. The local registrar's office reviews identification for accuracy and validity.
  5. The local registrar's office collects the required fee (as agreed in the Remote Issuance state contract).
  6. The local registrar's office has a choice of issuing a copy from their records or using the TxEVER Fee Module to issue a copy.
  7. If issuing from the TxEVER Fee Module, employee logs on to the system using their unique ID and password.
  8. Employee locates the requested record and issues a copy.

Election Identification

Note: EIC birth certificate can only be issued once during the customer's lifetime.

Remote Birth Access

Local registrar offices linked via the Remote Birth Access (RBA) system will use it for applicants requesting a birth certificate to obtain an Election Identification Certificate (EIC).

  1. Customer submits valid government-issued or acceptable primary identification to the local registrar's office.
  2. The local registrar's office reviews applications for accuracy, completeness, and eligibility.
  3. The local registrar's office reviews identification for accuracy and validity.
  4. The local registrar's office will not collect a fee for the record.
  5. Local registrar office employee logs on to the RBA system using their unique ID and password.
  6. The local registrar office employee locates the requested record and issues a certified copy of the election identification birth certificate.

Without a Remote Birth Access System

Local registrar offices will provide the long form record for applicants requesting a birth certificate to obtain an Election Identification Certificate (EIC).

  1. Customer accurately and completely fills out and submits an EIC Application for Election Identification Birth Certificate (PDF).
  2. Customer submits valid government-issued or acceptable primary identification to the local registrar's office.
  3. The local registrar's office reviews the application for accuracy, completeness, and eligibility.
  4. The local registrar's office reviews identification for accuracy and validity.
  5. The local registrar will not collect a fee for the record.
  6. Local registrar office employee locates the requested long-form record.
