Social Work Services - Public Health Region 4/5N

Medical Social Workers provide case management services, outreach and education, and client support services to children with special health care needs. These services are also available to high-risk women, infants, and children.

Social or Family assessment and intervention are available for all people with health problems. They also offer this to those who have trouble accessing health services. Social workers help with connecting medical, social, educational, and rehabilitative services between medical providers, the family, and the case manager. These services are identified in a plan of service developed between these groups. The medical social worker works in community-building and helping networks and partnerships. This work helps with resource development and coordination. Pediatric cardiology, orthopedic and craniofacial clinics are offered to children served by the Social Work Program. Parent groups that are supportive, educational, and encourage program participation are available in most communities.
