Social Work Services


Medical Social Workers provide case management services,outreach and education, and client support services to children with special health care needs and to high risk women, infants and children. Social/Family assessment and intervention is available for all individuals and families experiencing health problems or difficulty gaining access to health services.  Primarily, Social Workers assist with linkage to medical, social, educational and rehabilitative services identified in a Plan of Service developed cooperatively between appropriate medical providers, the family and the case manager. The Medical Social Worker works in the community building and helping networks and partnerships to assure resource development and coordination.  Pediatric cardiology, orthopedic and craniofacial clinics are offered to children served by the Social Work Program.  Parent groups that are supportive, educational and encourage program participation are available in most communities.
