Improve and maintain the health of the population of Region 6/5 South.
The purpose of Community Health Services is to provide population-based services to counties in Health Service Region 6/5 South not served by a local health department. The Community Health Team is composed of four programs in the region including the Community and Family Health Program, Contract Management Services, Oral Health Program, and the Tobacco Prevention and Control Program. The Community Health Team works in an integrated fashion to address the Essential Public Health Services in each county.
