Want to help even more children with special needs?

The Children with Special Health Care Needs (CSHCN) Services Program of the Texas Department of State Health Services (DSHS) wants you to be part of our team of providers! Help us help people with special needs from birth to age 21 years, and people of any age with cystic fibrosis.

Enrolling in the CSHCN Services Program is now a click of the mouse away! To enroll in the CSHCN Services Program, go TMHP's Enrolling in the CSHCN Services Program web page. You can enroll online or print out the paper application form to complete and return to TMHP. If you have questions, call your local area TMHP Provider Relations representative. To find your area representative, go to the Provider Enrollment Regional Support web page or call 1-800-568-2413. updated 8/5/2010

Since our providers have to be enrolled in Medicaid, some of the information from your Medicaid application will be auto-populated into your online application to the CSHCN Services Program, saving you time and effort.

Your online application is validated immediately, so you know that the online portion of your application is complete.

You need only to print out a few forms, finish those, and then mail them into TMHP for processing. After TMHP completes your enrollment, you will be able to reach even more children in need.
