ELIGIBILITY: Rural hospitals in 13 noncontiguous rural counties and census tracts: Angelina, Brown, Burnet, DeWitt, Dawson, all rural census tracts in Guadalupe County, Haskell, Maverick, Mitchell, Polk, San Augustine, all rural census tracts in Wichita County, and Young.
PURPOSE: The CHART Model is a voluntary opportunity for rural hospitals to test health care transformation supported by payment reform. It is a seven-year funding opportunity from CMS that requires hospitals to participate in multiple activities and reporting requirements. Additionally, hospitals that participate in the CHART Model will be able to take advantage of certain operational flexibilities that include certain Medicare benefit enhancements and beneficiary engagement incentives. Because HHSC as the Lead Organization for the CHART Model in Texas will be responsible for requesting operational flexibilities and selecting certain quality measures for reporting, it is important HHSC identify potential hospitals’ interest in participating in the CHART Model, so that it can begin to prepare hospitals for its role next year. To do this, HHSC requests you complete the form so that it can start working with hospitals that plan to participate, answer questions from those that need more information, and identify any available capacity for participation from hospitals that are no longer interested in the CHART Mode.
