Revisions to Forms for Requesting Texas Birth and Death Certificates and Corrections

The Texas Department of State Health Services (DSHS) announces updates to forms used to request vital records services. DSHS updated these forms to revise policies as well as improve customer services by providing clearer instructions and reducing the number of forms that customers must complete. 

The updated forms are effective November 1, 2018, and are available in the Vital Statistics Forms section at http://www.dshs.state.tx.us/vs/reqproc/forms.shtm.

Updates to vital statistics forms include the following.

  • Birth Certificate Application (VS-140): allows customers to order a long- or short-form birth certificate, an heirloom birth certificate, a birth verification, or a birth certificate for military personnel with current deployment orders.
  • Death Certificate Application (VS-142): will allow customers to order either a death certificate or a death verification.
  • Birth Certificate Correction Application (VS-170): will allow customers to make demographic corrections to a birth certificate as well as add, remove, or replace a parent. The form for correcting a birth certificate pursuant to an adoption (VS-160) remains unchanged.
  • Death Certificate Correction Application (VS-172): will allow an applicant to make demographic corrections to a death certificate.

Once updated forms are effective, old forms will only be accepted through December 31, 2018.
