The Laboratory Services Section offers the following types of Newborn Screening (NBS) kits: 

  • NBS3 – Test Kit Form for Medicaid, Charity Care and CHIP
  • NBS4 – Test Kit Form for all other (e.g., Commercial Insurance, Private Pay, etc.)

Through our internal Medicaid eligibility verification process, we have discovered that NBS test requests for some Medicaid eligible clients have been erroneously submitted on the NBS4 form. In these instances, effective for dates of service November 1, 2008 and forward, a credit will be issued by the DSHS Billing Branch to the laboratory customer’s account charged for the NBS4 kit.

Please be aware that the process to verify that a NBS test request submitted on an NBS4 card was in fact for a child that was eligible for Medicaid will require a minimum of six to nine months from date of service. Credits will not be issued until this process has been completed. To avoid the need for the issuance of credits to your account, please be sure to use the NBS4 cards only for infants covered by commercial insurance or other private pay mechanism and use the NBS3 cards for infants covered by Medicaid, CHIP or those needing testing under charity care.
