All divorces/annulments (with or without children) and other court cases or suits affecting the parent-child relationship must be reported through the clerk of the court to The Texas Vital Statistics Section. To meet the requirement, print the following form double-sided*:

*Form must be printed double-sided, on one sheet of paper, not two. Form is not for adoptions.
Consolidated reporting by petitioners, attorneys, and the courts is designed to make mandatory reporting more efficient, timely, and improve the quality of reporting. However, this reporting system is only as good or timely as you make it; therefore, your attention in completing and filing this report is critical.

Court of Continuing Jurisdiction
For information on the court of continuing jurisdiction (CCJ) of a child, contact the CCJ Registry at VSSRegistration@dshs.texas.gov. Inquiries should be submitted using form:

Todos los divorcios y anulaciones matrimoniales (con o sin hijos) y otros casos o demandas judiciales que afecten la relación padre-hijo deben reportarse a través del secretario del tribunal a la Sección de Estadísticas Vitales de Texas. *El formulario debe imprimirse por las dos caras, en una sola hoja de papel, no en dos. Este formulario no tiene validez para los casos de adopción.
La presentación de informes consolidados por parte de los peticionarios, los abogados y los tribunales tiene por objeto hacer más eficiente y oportuna la presentación de los informes obligatorios y mejorar la calidad de estos. Sin embargo, el hecho de que este sistema de información sea bueno u oportuno depende principalmente del usuario; por lo tanto, el cuidado que se ponga en completar y presentar este informe es fundamental.
