2025 TPCO State Needs Assessment Survey

The Texas Primary Care Office (TPCO) invites you to take 10 minutes to complete a brief survey and share your insights to identify unmet primary care needs, health workforce shortages, and barriers to care in our communities.

Your responses will be incorporated into the TPCO’s Statewide Health Needs Assessment Report and will serve as the basis for TPCO’s Primary Care Long Term Strategic Plan.  All responses will be confidential.

Deadline: September 26, 2025
Who should respond: Health care providers, public health professionals, and representative from community health centers,  Area Health Education Centers, public health community organizations, and health care professional organizations.
