Texas Cancer Registry Aggregate Data Request Form

Use this form when requesting aggregate cancer data from TCR. The Cancer Epidemiology and Surveillance Branch will respond by email, confirming receipt and asking additional questions, if applicable, typically within 2 business days. For questions about the form, email CancerData@dshs.texas.gov.

(Briefly describe how the data will be used.)

(examples: incidence counts/rates, mortality counts/rates, prevalence, relative survival)

(examples: all malignant cancer sites, female breast, prostate)
(examples: all Texans, or a specific sex, race/ethnicity, or age group)
(examples: Texas statewide, specific county. Note, the smallest unit available for aggregate data requests is the county level.)
(Note, The earliest diagnosis year that can be requested is 1995 for incidence and 1990 for mortality.)
(With some exceptions, requests for aggregate data generally require at least two weeks to complete.)
