Complete the Hansen's Disease Consultation Request form to connect with a DSHS Recognized Hansen’s Disease Medical Consultant. Annie Quratulain Kizilbash, MD, MPH
Dr. Kizilbash is an Associate Professor of Medicine at the University of Texas at Tyler Health Science Center and is the Medical Director of the Texas Center for Infectious Disease. She completed her residency in Internal Medicine at Emory University Hospital and her Fellowship in Infectious Diseases at Baylor College of Medicine. She received her Master’s in Public Health from the University of Texas School of Public Health. She has been managing patients with tuberculosis (TB) and Hansen’s disease since 2013.

Adriana Vasquez, MD, FACP
Dr. Vasquez is an Associate Professor of Medicine at the University of Texas at Tyler Health Science Center and physician at the Texas Center for Infectious Disease. She completed her residency in Internal Medicine at Woodhull Medical Center and her Fellowship in Infectious Diseases at New York University Medical Center. She has been managing patients with TB and Hansen’s disease since 1995.
