Texas Health and Human Services enterprise employees may use this form to request books or journal articles. You can also email the information to library@dshs.texas.gov or call 512-776-7559 (Austin) or toll-free 1-888-963-7111 ext. 7559.

Please list one book or article citation in each entry window below. Please be as specific as possible. Include, as applicable, identification number, title, author(s), date, journal name, volume number, issue number, page number(s). If you have more than 5 citation requests, please submit another form or email the information to library@dshs.texas.gov.
Articles: Please allow 2-5 days for delivery via email.
Books: Please allow as much lead time as possible. A minimum of 2 weeks is generally required.
