Vaccine Information Statements
The 89th Legislature Regular Session passed House Bill (H.B.) 4535.
DSHS developed a standardized COVID-19 vaccine information sheet in compliance with H.B. 4535. Health care providers should provide the vaccine information sheet to individuals when obtaining informed consent and administering the COVID-19 vaccine starting on September 1, 2025.

Vaccine Information Statements
Vaccine Information Statements (VIS) are sheets with information made by the Centers for Disease Control and Prevention (CDC) and DSHS. They inform individuals getting vaccines, their parents, or legal representatives about the benefits and risks of a vaccine.
Because of the National Childhood Vaccine Injury Act (NCVIA), federal law says that VIS must be given when certain vaccines are given. State law also requires that the state VIS sheet must be provided when a COVID-19 vaccine is given. You can find how to use VIS and keep records on the CDC's website.
Note: Federal law says you can use the federal VIS for six months after it's updated unless the CDC says to use the new one immediately.
