2025 Annual Survey of Hospitals DSHS/AHA/THA Annual Survey of Hospitals

All Texas general and special hospitals are required to submit updated information to the Department of State Health Services (DSHS) as part of the joint 2025 Annual Survey of Hospitals–Annual Statement of Community Benefits (ASH‑ASCB).

This annual survey is required by Texas law and collects aggregate financial, utilization, staffing, and other hospital data pursuant to Texas Health and Safety Code, Chapter 311, Subchapters C and D. The survey is conducted cooperatively by the Texas Department of State Health Services, the American Hospital Association (AHA), and the Texas Hospital Association (THA). Accurate and timely completion of the survey is essential for meeting statutory reporting requirements.

New Reporting Requirements for 2025

Recent legislation introduced new or revised reporting requirements that will be collected through the annual survey process:

  • House Bill 2854 (89R) revised Texas Government Code Sec. 508.193 to require DSHS to collect and maintain a list of the chief law enforcement officers for each general hospital. Hospitals that do not employ law enforcement officers must instead identify the local law enforcement agency with jurisdiction over the hospital location.
  • House Bill 2187 (89R) revised Texas Health and Safety Code, section 257.005, to require the Chief Nursing Officer (CNO) of each general or special hospital to attest to the accuracy of nurse staffing information submitted to DSHS. The survey will collect contact information for the CNO, who will be asked during the survey data verification period to attest to the accuracy of the hospital’s responses.

Hospitals are encouraged to review these statutory changes carefully to ensure that the required information is submitted and attested to by the appropriate individuals.

Survey Availability and Submission Deadline

The 2025 ASH‑ASCB survey is now available for online submission. Hospitals are required by rule to submit completed survey data within 60 days of receipt of this notification.

The submission deadline is July 13, 2026. No extensions will be granted.

 Hospitals that fail to submit required data may be subject to referral to the Texas Office of the Attorney General.

Survey Platform Updates

Over the past year, DSHS and AHA have updated the survey platform to improve usability and performance. Notable enhancements include:

• Secure single sign‑on registration for users without existing AHA accounts  

• User‑managed passwords and password resets  

• Ability to assign helpers to specific survey sections  

• Automatic data saving  

• Faster system performance and improved progress tracking
