We look forward to welcoming Jacky Sylvie, Director of Community Outreach with Finding Independence by the Community for Permanent Supported Housing who will present “Transition to Adulthood: Navigating and Funding Housing Options for People with Disabilities.” Jacky will share her expertise on housing options, affordable housing resources, financial resources, safety products, and more! Finding Independence provides support with planning, searching, and securing safe, affordable homes to help people with disabilities thrive.

Transition to Adulthood Learning Collaborative

The Transition to Adulthood Learning Collaborative (TALC) meets quarterly via webinar. A guest speaker presents on a transition topic. We also share information on upcoming events and new transition resources. All are welcome to join us, including:

  • Youth and young adults;
  • Parents and caregivers;
  • Case managers;
  • Health care plan representatives;
  • School transition specialists and teachers;
  • Hospital and university systems, and
  • Local community organizations.
