We’re excited to introduce our new, user-friendly survey-based reporting interface, designed to be easier to navigate. VHSS Reporting Season for School Year 2025-2026 transitioned to the Vision, Hearing, and Spinal Screening Annual Reporting Survey (VHSSARS).

On or before June 30 of each year, each facility shall submit to DSHS a complete and accurate annual report on the vision, hearing, and spinal screening status of its aggregate population screened during the reporting year. Facilities shall report in accordance with the department's specifications. The reporting period for submitting your reports is January 15 to June 30 each year.

The Vision, Hearing, and Spinal Screening (VHSS) Reporting System is a survey-based system that provides users with an easy-to-use way to submit their annual reports. The VHSS Annual Reporting Survey PowerPoint includes step-by-step instructions for entering your annual reports in Qualtrics VHSSARS.

Each academic year, the Vision, Hearing, and Spinal Screening Annual Reporting Survey (VHSSARS) link will be active only during the reporting period. Outside of the reporting period, the link will be deactivated until January 15. Each reporting period, a new link will be provided on the VHSS webpage. Be sure to subscribe to the website to receive the most current Announcements and Updates.

BEFORE YOU REPORT

Facilities are encouraged to have their summary totals ready in advance to speed up the process and to verify them to confirm they are correct. 

You may request data instruction PDFs, which explain each survey column, and an Excel spreadsheet to track aggregate data by emailing the program. Be sure to print a copy of your submission for your records. 

You will need your Facility ID before you begin entering your data.

The Facility ID assigned to a facility helps distinguish and identify each individual facility type during the reporting period. 

If you do not know your Facility ID, please include the following information in your email to vhssprogram@dshs.texas.gov so we can assist you, or you may contact your Regional Coordinator to request that information.
