Contact Information for Privacy Concerns and Issues
HIPAA Privacy and Security Rules apply to covered entities. A covered entity is a health plan, health care clearinghouse, and any health care provider that conducts certain health care transactions electronically.
If you believe that a covered entity violated your (or someone else's) health information privacy rights or committed another violation of the Privacy or Security Rules, you may file a complaint with the Health and Human Services Office of Civil Rights (OCR). OCR is the federal entity that enforces HIPAA. Their website is located at: http://www.hhs.gov/ocr/hipaa/ and the hotline is 1-866-627-7748 (voicemail).
