School and Childcare Vaccine Requirements

Texas Capitol Building surrounded by treesSchool and childcare vaccine requirements are authorized under Texas statutes, Health and Safety Code, Education Code, and Human Resources Code. 

In September 2025, the process for obtaining a conscientious exemption was modified due to the passage of House Bill 1586, 89th Legislature, Regular Session.  The bill amended Health and Safety Code Chapter 161.0041, which states that a person claiming an exemption from a required immunization based on reasons of conscience, including a religious belief, must complete the blank affidavit form provided by DSHS.  Additionally, DSHS revised the Texas Administrative Code, Title 25, Chapter 97, §97.62 Exclusions from Compliance and §97.64 Required Vaccinations for Students Enrolled in Health-related and Veterinary Courses in Institutions of Higher Education.

DSHS now posts a blank immunization exemption affidavit form, stock F11-11755 (PDF) for a person to download and submit to their child-care facility, school, or institution of higher education, including health-related or veterinary courses. A person can still request that DSHS mail them the form.
