A Potentially Preventable
Readmission (PPR) is a readmission (return hospitalization) to an acute care
hospital following a prior acute care admission within a specified time
interval, for a reason that is clinically related to the initial
hospitalization.
The
Department of State Health Services (DSHS) is required by Section 98.1046, Texas Health and Safety Code, to
publicly report information on Texas hospitals regarding risk-adjusted outcome
rates for PPRs. The outcome rates are determined by DSHS to be the most
effective measures of quality and efficiency. In publishing the report, DSHS
may not disclose the identity of a patient or health care professional.
PPRs are
increasingly viewed as an indicator of healthcare quality, based on the
expectation that PPRs could have been prevented with quality care during the
initial admission, adequate post-discharge planning and follow-up, or improved
coordination between inpatient and outpatient healthcare services.
