Forms & Instructions

The DSHS IRB review process has three stages :

  1. Program Approval
  2. Institutional Review Board Approval
  3. Research Executive Steering Committee Approval (Initial Applications only)

Access to the requested data/biospecimens is permissible only after receiving approval at each stage and submitting an executed MOU/DUA agreement.

Follow the steps below to submit an application:

  1. Contact the DSHS IRB program that holds the desired data/biospecimens  ( Alphabetical List of Programs and Contacts). Discuss the intended project and determine the best way to present your request. Commonly requested data comes from the following programs:
  2. If the program determines an IRB application is required, complete and submit all IRB application materials directly to the program. A program contact will be assigned to you. Follow their instructions for the program submission procedures. Include any additional materials to ensure a substantial IRB review process.

  3. Please reference the appropriate IRB Submission Checklists to help gather the appropriate forms for your submission type (i.e., Initial, Renewal, Amendment, Renewal with Amendment).

  4. All IRB requests require an executed MOU before data/biospecimens can be released. Please review the MOU template. Identify a person authorized to sign the agreements. Provide their contact information on the HRP 301 application form under the authorized signatory section.

  5. Sign and date the HRP 301 application form. Send the complete submission to the program contact. The program contact will obtain all required approvals and forward the submission to the IRB Office.

Submission Deadline

To ensure the program and IRB staff have sufficient time to review the submission, please submit applications to the program contact at least 10 weeks before the IRB meeting submission deadline or IRB approval expiration date. 

Reference the Meeting Schedule & Submission Deadline for specific meeting dates.

Submissions that require full board review must be submitted to the IRB by the meeting submission deadline. Applications received after the submission deadline will be held until the following meeting for review. Applications that meet requirements for an expedited review will be processed on a rolling basis.
