The information in this section is intended to provide public notice of disciplinary action by the Department of State Health Services and the Division for Regulatory Services, Compliance Unit and is not intended to reflect the specific findings of either entity.

This information may not reflect any number of factors including, but not limited to, the severity of harm to a patient, any mitigating factors, or a provider/certificant's disciplinary history. This listing is not intended as a guide to the level of sanctions appropriate for a particular act of misconduct.

Brown, Barbara, Canton, TX.  January 8, 2019, revoked for violating EMS Rules 25 TAC §§ 157.36(b)(24), 157.36(b)(26), 157.36(b)(30), and 157.36(b)(44) related to arrests for driving while intoxicated, forgery financial instrument, failure to notify the Department of current mailing address, and failure to provide requested information to the Department.
Brummett, Paige, Bonham, TX.  April 3, 2019, 6-month suspension for violating EMS Rules 25 TAC §§ 157.36(b)(3), 157.36(b)(8), 157.36(b)(31), 157.36(b)(35), 157.36(b)(36), 157.36(b)(40), and 157.36(b)(41) related to failure to follow medical director’s protocols, accurately document PCRs, and engaged in conduct that has the potential to jeopardize the health and safety of patients.
Cannon, Debbie A., Tatum, TX.  June 12, 2019, revoked for violating EMS Rules 25 TAC §§ 157.36(b)(23) and 157.36(b)(24) related to voluntarily surrendering her Registered Nurse and Vocational Nurse Licenses in lieu of pending disciplinary action regarding allegations of misappropriating medication from her employer and/or patients, testing positive while on-duty for Hydrocodone, and failure to provide requested information to the Department.
Carson, Chelsea, Lubbock, TX.  June 26, 2019, 7-day suspension beginning on July 26, 2019 for violating EMS Rules 25 TAC §§ 157.36(b)(2), 157.36(b)(3), 157.36(b)(4), 157.36(b)(31), 157.36(b)(35), 157.36(b)(41), and 157.36(b)(47) related to failure to protect and/or advocate for a patient /client from unnecessary risk of harm and falsified patient care report.
Castillo, Jr., Juan Jose, Alamo, TX. March 23, 2018, 6 day suspension beginning April 23, 2018; 3 year, 11 months, 24 days probated suspension for violating EMS Rules 25 TAC §§157.36(b)(2), 157.36(b)(6), 157.36(b)(12), 157.36(b)(19), 157.36(b)(24), 157.36(b)(26), 157.36(b)(31), 157.36(b)(32), and 157.36(b)(35) related to an arrest for public intoxication, failure to notify the Department of an arrest, failure to provide requested information to the Department, and failure to comply with probationary conditions outlined in an Agreed Order.
Cellio, II, Thomas A., Frisco, TX.  September 20, 2018, assessed an administrative penalty of $1,000.00 for violating EMS Rule 25 TAC § 157.43 (m)(3)(AA) related to demonstrating unprofessional conduct.
Chambers, Michelle D., Spring, TX.  February 19, 2019, revoked for violating EMS Rules 25 TAC § 157.36(b)(24) related to failure to provide requested information to the Department.
Chapa, Antony R., Alto, TX.  July 29, 2019, 5-day suspension beginning on August 29, 2019, for violating EMS Rules 25 TAC § 157.36(b)(40) related to failure to contact medical control before administering Toradol for pain management.
Christopher R. Freed DBA North Texas EMS, Fort Worth, TX.  April 10, 2019, assessed an administrative penalty of $10,000.00 for violating EMS Rule 25 TAC §§ 157.16(d)(12), 157.11(n)(16), 157.16(d)(19), and 157.11(n)(26) related to operating without medical director oversight, failure to notify the Department of a change in its medical director status, and failure to provide requested information to the Department.
Cline, Michael J., Gainesville, TX. December 3, 2018, 60-day probated suspension for violating EMS Rules 25 TAC §§157.36(b)(2), 157.36(b)(16), 157.36(b)(17), 157.36(b)(21), 157.36(b)(22), 157.36(b)(31), 157.36(b)(35), and 157.36(b)(47) related to misappropriating controlled substance from patient, illegally possessing and self-administering controlled substance, and failure to follow medical director’s protocols.

Copeland, Michael B., Millsap and/or Aledo, TX.  January 18, 2019, revoked for violating EMS Rules 25 TAC §§ 157.36(b)(23), 157.36(b)(24) and 157.36(b)(30) rules in effect on the date of the occurrence, related to arrests for aggravated assault date/family/house with a weapon, 2 counts of possession of child pornography, failure to notify the Department of an arrest, and failure to provide requested information to the Department.
Cotton, Brent A., Athens, TX.  February 26, 2019, 6-month suspension for violating EMS Rules 25 TAC §§ 157.36(b)(2), 157.36(b)(16), 157.36(b)(17), 157.36(b)(22), 157.36(b)(31), 157.36(b)(33), and 157.36(b)(35) related to misappropriating controlled substance from employer, self-administering controlled substance, and testing positive for controlled substance without proper authorization from a physician’s prescription and/or employer’s medical director.
Cousins, Grant A., Crandall, TX.  March 7, 2019, 12 month suspension with the initial ten (10) day actual suspension beginning April 6, 2019; eleven (11) months, twenty (20) days probated suspension for violating 25 TAC §§157.36(b)(2), 157.36(b)(29), 157.36(b)(35), and 157.36(b)(47) related to an arrest and deferred adjudication for Perjury, and making a false statement under oath to a Texas Ranger.
Cox, Thomas W., Jacksboro, TX.  August 13, 2019, revoked pursuant to Texas Occupations Code § 53.021(b) related to felony conviction and imprisonment for theft of property.
Cross, III, Marvin L., Burleson, TX.  June 24, 2019, revoked for violating EMS Rules 25 TAC §§ 157.36(b)(2), 157.36(b)(24), 157.36(b)(25), 157.36(b)(26), 157.36(b)(29), 157.36(b)(30), 157.36(b)(31), and 157.36(b)(35) related to an arrest and conviction for Driving While Intoxicated 3rd or more, failure to notify the Department of an arrest and change in criminal history, and failure to provide requested information to the Department.
Douglas, Christopher R., Whitewright, TX.  March 6, 2019, 3-day suspension for violating 25 TAC §§157.36(b)(31), 157.36(b)(35), and 157.36(b)(41) related to failure to protect and/or advocate for patient and/or public from unnecessary risk of harm when partner displayed signs of impairment and drove ambulance which resulted in collision.
Emergency Medical Training Services, Frisco, TX.  September 20, 2018, reprimand for violating EMS Rules 25 TAC §§ 157.32(u)(2)(J) and 157.32(u)(2)(K) related to failure to maintain professionalism and lack of supervision of course coordinators.
Emergency Training Enterprises, Sour Lake, TX.  January 2, 2019, 24-month suspension with the initial 12-months actual suspension and remaining 12-months probated suspension for violating EMS Rules 25 TAC §§ 157.32(t)(3), 157.38(g)(1), and 157.38(g)(2) related to failure to hold current approval to conduct CE courses and awarding CE hours.
Faust, Aidan, San Antonio, TX.  December 13, 2018, revoked for violating EMS Rules 25 TAC §§ 157.36(b)(2), 157.36(b)(22), 157.36(b)(24), 157.36(b)(25), 157.36(b)(26), 157.36(b)(29), 157.36(b)(30), 157.36(b)(35), and 157.36(b)(44) failure to provide requested information to the Department, an arrest and conviction for possession of marijuana, an arrest and deferred adjudication for possession of controlled substance, failure to notify the Department of arrests, and failure to notify the Department of a change in criminal history.

March 8, 2017, 36 month suspension; 3 months actual suspension followed by 33 month probated suspension for violating EMS Rules 25 TAC §§157.36(b)(2), 157.36(b)(14), 157.36(b)(19), 157.36(b)(28), 157.36(b)(29), and 157.36(b)(30) related to tampering, adulterated, misappropriated and/or self-administered Fentanyl from employer. 
Fithian, Joshua D., Fate, TX. and/or Newburgh, IN.  October 10, 2018, revoked by operation of law, see Tex. Health & Safety Code § 773.0614(c) related to a deferred adjudication for indecency with a child exposes.
Froh, Samuel, Princeton, TX.  August 15, 2019, 12-month probated suspension for violating EMS Rules 25 TAC §§ 157.36(b)(31), and 157.36(b)(36) related to use of a contaminated intravenous (IV) catheter on a patient and failure to maintain a substantial amount of skill, knowledge and/or academic acuity to timely and/or accurately perform duties and or meet responsibilities required.
German, James M., Conroe, TX.  February 19, 2019, 3-day suspension beginning March 19, 2019 for violating EMS Rules 25 TAC §§ 157.4(e), 157.34(b)(2)(A), 157.36(b)(2), 157.36(b)(18), 157.36(b)(21), 157.38(c), and 157.38(k)(5) related to failure to provide appropriate documentation and/or proof to validate attestation and/or CE compliance. 
Glover, Alan P., Dallas, TX.  September 26, 2018, revoked for violating EMS Rules 25 TAC §§157.36(b)(24), 157.36(b)(26), and 157.36(b)(30) related to an arrest for driving while intoxicated, failure to notify the Department of an arrest, and failure to provide requested information to the Department.
Gordon, April, Sour Lake, TX.  June 11, 2019, 15-day suspension beginning on July 11, 2019 for violating EMS Rules 25 TAC §§ 157.36(b)(20) and 157.36(b)(47) related to academic dishonesty by cheating on an Operations Exam.

Lee, Brandon A., Sour Lake, TX.  January 2, 2019, assessed an administrative penalty of $1,600.00 for violating EMS Rule 25 TAC §§  157.43(m)(3)(B), 157.43(m)(3)(F), 157.43(m)(3)(N), and 157.43(m)(3)(T) related to failing to comply with the responsibilities of a course coordinator and unlawfully continued to offer CEs.
Lozano, Rodrigo, Eagle Pass, TX.  July 31, 2019, deny initial application for EMT certification due to regulatory authority in 25 TAC §§ 157.36(c)(1), 157.36(c)(3), 157.36(c)(5), and 157.37(e)(1)(A) related to an arrest for possession of marijuana and possession of a dangerous drug, convictions for possession of marijuana, driving while license suspended, driving while intoxicated, deferred adjudication for aggravated assault with a deadly weapon, and failure to disclose on your initial application for EMT certification.
